Birth plan checklist
Your preferences, organized — and printable for the care team.
What it does
The Collaborative Birth Plan Studio turns "we should write a birth plan" into a checklist you can actually finish. Browse preference categories (or start from a template: Hospital Natural, Gentle C-Section, Birth Center / Home, Pain Management Focus), mark each preference as Must-Have, Preferred, or Avoid/Veto, and add personalization: baby's name, due date, delivery location, OB/midwife, doula, and special medical notes. It syncs live with your partner or doula by link or QR code, exports/imports JSON backups, and generates a printable Labor & Delivery Birth Plan Manifest for your care team.
Who it’s for
Parents who want their preferences on one calm page the care team can actually read — and partners and doulas who should be able to see the same plan.
How to use it, step by step
- Start from a template that matches your birth setting, or begin with the full Preferences Checklist.
- Search preferences (try "epidural" or "music") and mark each as Must-Have, Preferred, or Avoid.
- Fill in the Personalization tab: names, dates, hospital, care team contacts, and medical notes.
- Share the live link or QR code with your partner or doula so everyone edits the same plan.
- Print the Birth Plan Manifest and put a copy in the hospital bag.
What you get
A prioritized preference list (Must-Haves / Preferred / Avoid), care-team details, and a printable birth plan document.

💡 Tips & tricks
- Use the JSON export as a backup before the birth — and import it on your partner's phone so the plan exists on two devices.
- The Avoid/Veto column is as valuable as the Must-Haves; care teams consistently say clear "no thank you" items help them most.
Enough reading — the tool itself is one tap away, and it is free.
