Starting 1 October 2026, most of the continental United States enters the RSV immunization window. If you are delivering this month, or you have an infant who has not had a dose and whose parent was not vaccinated late in pregnancy, this week is the practical moment to ask.

Background
RSV is the leading cause of hospitalization in US infants. Since 2023, two tools have been available: a maternal vaccine given in late pregnancy, and a long-acting monoclonal antibody given to the infant (nirsevimab or clesrovimab). One path is usually enough.
AAP expanded second-season eligibility this year for some higher-risk toddlers. That policy change is already on this site. What is new this week is the calendar: the window opens.
What changed this week
For most of the Lower 48, 1 October is the date hospitals and clinics start treating RSV immunization as in-season. Birth hospitals that stock the antibody can give it before discharge. Pediatric offices that participate in Vaccines for Children can order doses, but allotments still vary. Some hospitals have spent September arguing with insurers about how to bill a nursery dose.
If you deliver after 1 October and nobody mentions RSV before you leave, ask: will this baby get an RSV antibody before discharge, or do we need an appointment?
Competing viewpoints
Hospital teams that stock the product argue the birth dose is the only reliable way to cover the first weeks at home. Clinic-first teams argue inventory is tight. Insurers have treated nursery billing as a new line item.
