Pennsylvania’s fifth measles-associated death, and why the CDC still says two

Pennsylvania reported a fifth measles-associated death on 30 September 2026: an unvaccinated Lancaster County resident. The state now counts 943 confirmed cases across 39 counties and 176 hospitalizations. The CDC latest published tally has confirmed two measles deaths nationwide and 3,659 cases. That gap is the parent question of the week.

Background

The United States declared measles eliminated in 2000. Since 2025, large outbreaks have returned. The CDC 2026 count, cited by the AAP Red Book update of 30 September, stood at 3,659 confirmed cases as of 24 September, with about 95 percent unvaccinated or of unknown status, and about 18 percent of cases in children under 5. Pennsylvania is the hardest-hit state. Two earlier Lancaster County deaths in August were the first reported in the US this year, followed by deaths in Jefferson and Mifflin counties in mid-September.

Infants under 12 months are not routinely given MMR. The first dose is usually at 12-15 months, the second at 4-6 years. During an outbreak, clinicians sometimes offer an early dose from 6 months. That early dose does not count as the full series.

What changed

On 30 September the Pennsylvania Department of Health said it had reported the Lancaster County death to the CDC. The person was unvaccinated and had tested positive for measles. The department said this is the fifth Pennsylvanian who meets its measles-associated death definition. It also said more than 53,000 MMR doses were given in September, more than double a typical month, and that only four of 943 cases were in vaccinated people. The department cites two doses of MMR as about 97 percent effective at preventing measles.

Reuters reported the same day that the CDC had confirmed only two measles-related deaths nationwide as of Tuesday. AAP Red Book, updated 30 September, still points to the 3,659-case figure as of 24 September, with 327 hospitalizations (9 percent).

Competing viewpoints

Pennsylvania counts measles-associated deaths: the person had confirmed measles and died. Governor Josh Shapiro administration has argued the national tally should reflect those state reports.

The CDC has moved toward National Center for Health Statistics mortality data instead of rapid state reports. Health Secretary Robert F. Kennedy Jr. has said only one of the Pennsylvania deaths was caused by measles. Supporters of the narrower count argue association is not the same as cause.

Clinicians such as Amesh Adalja at Johns Hopkins have called each of these deaths preventable. The AAP still recommends the routine two-dose MMR schedule and an early dose for infants 6-11 months during an outbreak. Critics of federal messaging say mixed statements make clinic conversations harder. Critics of state messaging say a broader associated label can overstate what a death certificate will eventually show.

What parents can use

This is not a diagnosis and not a substitute for your pediatrician.

  • Ask whether each household member has two documented MMR doses, or evidence of immunity.
  • If you are pregnant, have a baby under 12 months, or care for someone who cannot be vaccinated, ask the clinic what to do if a local case is announced.
  • Infants 6-11 months may be offered an early MMR dose during an outbreak. Confirm whether that dose is extra.
  • Fever plus cough, runny nose, red eyes, then a rash that starts at the hairline is the classic pattern. Call ahead before walking into a waiting room.

What to watch next

Whether the CDC accepts Pennsylvania fifth death into the national tally. Whether the September MMR surge of 53,000 doses shows up as fewer new cases in October. Whether the US measles-elimination review treats 2026 as continuous transmission.
