Children 6 months through 8 years who have not already received two lifetime influenza doses usually need two doses this season, at least 28 days apart. Aim to finish both before Halloween if the clinic has product.
Write-in card fields
- Child first name
- Date of birth
- Clinic or pharmacy name and phone
- Dose 1: date / lot / site
- Dose 2 target: dose 1 + 28 days, plus actual date
- Checkbox: VFC / insured / self-pay (informational only)
