BabyCCINO: Finding the Right Caffeine Dose for Preterm Babies
A landmark trial launched this week in Melbourne is set to answer a long-standing question in neonatal care: what is the optimal dose of caffeine for very premature infants?
Background from the CAP trial 20 years ago showed caffeine reduces apnea and improves long-term outcomes, but doses vary widely across hospitals. The new BabyCCINO study, part of the PLATIPUS platform, will compare high, medium, and low regimens in ~3900 babies across Australia, New Zealand, and the UK.
What changed: Researchers led by Prof Brett Manley at Mercy Hospital for Women are randomizing infants under 32 weeks to different loading and maintenance doses, tracking hospital outcomes and development 2-3 years later.
Competing viewpoints: Some units prefer higher doses for stronger respiratory support; others worry about side effects and stick to lower ones. No consensus exists yet.
What to watch: Results expected in coming years could standardize care worldwide and further reduce complications like bronchopulmonary dysplasia.
Parents of preterm babies should discuss caffeine with their neonatologist; this trial aims to make that advice more precise.
